Current ESC/ERS Guidelines and the proceedings of the 7th World Symposium on Pulmonary Hypertension recommend initial treatment with triple combination therapy including a parenteral prostacyclin for patients at high risk and may be considered in non-high risk with severe haemodynamics and/or poor RV function. However, it is well acknowledged that there remains a need for improved utilisation of existing therapies, particularly parenteral prostacyclin pathway agents. In current clinical practice, a vast majority of patients still do not receive optimal treatment when considering drug combinations, timing and dosing, although well-conducted observational studies have suggested significantly improved outcomes, including survival, for initial triple combination therapy including a parenteral prostacyclin in high and intermediate-high risk patients. The aim of this lecture is to increase the acceptance, timing and dosing of initiation of parenteral prostacyclins, with an update of the evidences and recommendations.
